Clinical Pharmacist

Responsibilities and duties:

· Provides a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient’s medication use to achieve the best drug utilization.

· Performs daily rounds on hospitalized patients and review patient profile, analyze all laboratory data and record all required patient information to identify, prevent, or mitigate drug-related problems.

· Communicates & consults with the physicians and other health care providers to develop & implement the optimum medication plan.

· Makes appropriate evidence-based patient-centered medication interventions.

· Designs and implements stewardship activities and restriction/surveillance programs & ensures the appropriate compliance of healthcare staff to them.

· Develops and ensures implementation of medical & therapeutic protocols related to medications use in the hospital.

Requirements:

· Bachelor’s Degree in Pharmaceutical Sciences

· Pharm-D or Board of Pharmacy Specialties (BPS).

· 1 -3 years of experience in the same position ( preferred in private hospitals)

· Excellent Pharmacological knowledge of medications (indications, dosing, precautions, drug interactions, adverse reactions & side effects).

· Team player, outstanding communication & organization skills.

· Strong Computer skills.
